Human Resources provides employment verification for previous and current employees by completing sections 3 and 4 of the Public Service Loan Forgiveness (PSLF) application. The application can be submitted via the PSLF Help Tool (DocuSign), emailed to hrservices@louisville.edu, or dropped off at Human Resources. Please note: It may take up to three business days for Human Resources to return your application once it’s been submitted.
Before submitting the application, doublecheck the Federal Employer Identification Number (FEIN) listed. The FEIN for the University is 61-1014882.
If you are an employee of the University of Louisville Hospital/Physicians Group/Health, you will need to submit your PSLF form to the hospital’s HR office at humanresources@uoflhealth.org.
